Nose Dive starts with a command to turn the Game Boy Advance 90 degrees rclockwise, as it is a vertical shoot 'em up game. The player controls the robot Handroid V. They automatically move back and forth across the bottom of the screen, slowing down to turn at the edges of the screen. They move farther off the left edge of the screen before changing direction Handroid V holds out their right hand with two fingers extended. They shoot their hand upward as a hand missile. when the player presses {{button|GBA|A}}, generating a new hand afterward. Enemies are noses, which descend from the top of the screen in various formations and different movement patterns If Handroid V touches a nose, the game ends. The hand missiles defeat noses on contact, but have a few effects based on how they hit. Hitting the nose with one of the fingers causes it to scrunch up and explode or get knocked away and explode. Getting one finger in a nostril of a nose causes the hand missile and nose to veer off screen in the direction of the nostril that was not picked, defeating any additional noses that either entity hits. Successfully putting both fingers of a hand missile in a nose has the hand missile push the nose right off the top of the screen, with any nose above the nose getting pushed along and away as well.

After defeating all the normal noses, Handroid V stops in the center of the screen for a cutscene where a big face with two eyes and a bigger nose. It must be fought to proceed. This nose does not move, but it does blow wind out from its larger nostrils and occasionally drop a booger as a projectile. The booger negates one hand-missile, while the wind makes hand missiles move slower after they move from Handroid V while roughly in line with a nostril. To defeat the big nose, the player has to block its two nostrils by shooting a hand missile into each of them. All other hand missiles are ignored. They can hit the rest of the nose, hit the face, or land in a nostril that is already blocked, but they all bounce off and explode. Blocked nostrils cannot blow wind or drop boogers. The nose explodes when both nostrils are plugged. For this sequence, the music changes to become more intense.

After the bigger nose is defeated, another cutscene begins that has Handroid V stop in the center. The eyes of the face fall away and the face drops down. The face is revealed to be a even bigger nose. For this phase, Handroid V eats a fruit that powers it up, enabling it to hold out both hands. For this phase, instead of shooting hand missiles, pressing {{button|GBA|A}} makes Handroid V stop to rise straight up and pick the giant nose itself. To defeat the giant nose, Handroid V has to successfully get one of its hands into the giant noses nostrils a certain amount of times. Handroid V bounces off the parts of the nose that are not nostrils, and returns to the bottom of the screen after performing an attack regardless of if it connected or missed entirely. After returning, Handroid V resumes normal movement. Handroid V no longer scrolls farther off the left side of the screen than the right.

This giant nose moves back and forth on the top of the screen the screen in a sine wave pattern, slowing down to reverse at the edges of the screen. Sometimes it stops to send out noses from the nostrils which again descend to Handroid V. These noses always drop straight down, and may come from one nostril or both nostrils. Handroid V's tackling attack can also defeat normal noses. If Handroid fails to pick both nostrils of a nose, then Handroid V bounces back. If Handroid V picks both nostrils of a nose, then Handroid V pushes that nose upward and continues to rise, usually allowing Handroid V to knock out all other noses and pick the giant nose in one attack. If Handroid V delivers the final blow, it stops as the nose starts exploding. Handroid V flies off the top of the screen, pushing the giant nose off screen in the process. The player's final instruction is to rotate the Game Boy Advance 90 degrees counterclockwise, returning the system to normal. For this sequence, the music changes again, then becomes a victory fanfare after defeating the giant nose.

In ''WarioWare Gold'', the Boss Game has received many changes. As with all Boss Games in ''WarioWare Gold'', both screens are used as a continuous playfield. The player is not told to rotate the [[Nintendo 3DS]]. In practice, this does not meaningfully change the orientation of Nose Dive's playfield, but it does increase the size of the playfield relative to the characters. As vertical player movement is not involved until the boss battle, Handroid V remains on the Touch Screen for most of the game. Handroid V is redesigned to have a yellow visor. The player can use the left and right buttons to move Handroid V, as opposed to Handroid V moving left and right automatically. The boundaries that Handdroid V cannot move past as defined by ''WarioWare: Twisted!'' are still respected, though Handroid V cannot move as far off the left side of the screen as before. Handroid V takes two hits to be defeated instead of just one hit, gaining black marks across its body to indicate it has taken a hit. There is an active hitbox on Handroid V's hand, allowing Handroid V to defeat the noses by steering such that their hand touches the noses. Near the end of the normal nose phase, a container marked with Handroid V's head with a golden eye drop inside will scroll from one side of the Top Screen to the other. It does not descend. Hitting it with a hand missile causes the eye drop to fall. Catching the eye drop with Handroid V's head allows it to use a heat vision attack. This sends two lasers from the eyes that each sweep away from the center of the screen, eliminating all remaining noses. The fight against the big nose has been removed, so the game proceeds to the giant nose battle. Each level has a different giant nose to battle, with different abilities and different music. The first level has the normal giant nose, and retains ''WarioWare: Twisted!'''s music. Handroid V's default height is a little farther up the Touch Screen for this phases, and the giant nose is positioned about in the middle height of the Top Screen.
