The Sleep status ailment returns in Paper Mario: The Thousand-Year Door and its Nintendo Switch remake. It is indicated by multiple hovering "Z"s and a Sleepy Sheep icon above the afflicted character. The number by the Sleepy Sheep indicates how many turns the character will remain asleep if undisturbed. In these games, when sleeping characters are attacked, there is a chance that they will wake. This is signified by a small exclamation point over the character when they are attacked. If the player is inflicted with the sleep effect, they can still use Action Commands such as Guard and Superguard. The timing windows for these actions will be reduced while sleep is in effect, however. Crazee Dayzees, Amazy Dayzees, and Bonetail can all inflict this ailment with their attacks, Mario's party or an enemy with the Sleepy Sheep item can inflict Sleep on the opposing party, and Mario can also cause it through the Sleepy Stomp badge.

In the remake, the probability of being woken from Sleep early is changed in such a way that most enemy attacks, as well as all Mario and partner attacks, are no longer capable of ending Sleep early. This makes the status far more powerful.
