World 28 is a cloud world with floating platforms resembling Bouncy Cloud Boomerangs from New Super Mario Bros. U in Dr. Mario World similar to World 15, except it takes place at night. The first area is available as of 11 PM PT on May. 6, 2021[1], while the second area, challenge stages and special stage are available as of 11 PM PT on May. 20, 2021[2].

When an area is cleared, Paratroopas, Parabombs, and Foos appear. The viruses featured in the regular stages are red, green and light-blue.

Starting with 33 capsules, the objective of this stage is to eliminate all viruses (34) and collect all keys (3).

This stage introduces muddy coins, which are coins covered in mud that will require capsule matches next to them or direct elimination on them to remove the mud, which can require multiple matches to remove them. In this stage, there are small muddy coins in between the keys and doors, where the ones in the middle column each require two hits to clear, while the ones in columns #2 and #5 each require only one hit to clear. Viruses are in between the muddy coins for rows #5 to #8, floatie viruses are in between the muddy coins in #2, and ice is in between the muddy coins in rows #3 and #4. Some of the ice contain viruses and electric exploders.

The top contains a triangle of Empty Blocks, the sides contain cages where one cage on each side contains a lock block, and the bottom has cages below the crates and colorful crates.

For each group of viruses, eight of them are half-frozen, arranged in a checkered pattern.

In this stage, purple coins (caged and uncaged) are arranged in rows of four, where they are in between colorful crates and rows of viruses. Some of the rows of viruses have gaps in between, and some of the viruses are encased in ice.

Purple coins are unlocked in the following sequence:

  1. The 8 coins in rows #2 and #3.
  2. The 8 coins in rows #4 and #5.
  3. The 8 coins in rows #6 and #7.
  4. The 8 coins (initially half-frozen) in rows #8 and #9.

The capsule amount is increased from 25 to 29 since June 3, 2021. Besides that, this stage is otherwise the same.

In this stage, barrels are arranged in an L-shape and are surrounded by Brick Blocks, of which only the middle one in row #2 and the second on in row #7 are empty while the rest are Coin Blocks. All the barrels contain exploders. Shells are found on the rightmost two spots in rows #6 to #8, and bubbles containing viruses are found in the rest of rows #2 to #5. The topmost row contains barriers and two ice cubes containing a pop cannon on the left and a virus on the right.

The red lock block is in the left barrel, while the green lock block is in the right barrel.

All Brick Blocks contain hidden viruses, and some of those viruses have virus coins behind them. The filled flask in this stage will shoot out either viruses or exploders.

In this stage, dust clouds are separated into four quarters where they contain purple coins (caged and uncaged), keys, doors, and viruses. The two higher quarters are joined in the middle by a dust cloud containing a door. A fully frozen key is located directly below that dust cloud. Viruses are in between the two lower quarters, while a shell and Brick Blocks are in the row in between the groups of dust clouds. The two leftmost and two rightmost Brick Blocks contain doors.

Purple coins are unlocked in the following sequence:

  1. The 4 coins inside the lower-left group of dust clouds.
  2. The 4 coins inside the lower-right group of dust clouds.
  3. The 4 coins inside the upper-left group of dust clouds.

In this stage, the keys are in the leftmost and rightmost columns, and their corresponding doors are found in the last four revealed rows, where they are inside cages. At the beginning of the stage, there are colorful crates and viruses alongside the keys, where some of the viruses are half-frozen or inside bubbles. Muddy coins are found starting from the first-revealed row, and together with colorful crates, are found until the ninth-revealed row. Floatie viruses are found starting from the fifth-revealed row. The lock blocks are found in the eighth- and ninth-revealed rows. Brick Blocks (all containing hidden viruses) are found starting from the tenth-revealed row where exploders are also found. Some of the objects throughout the stage are encased in ice.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 13 lines are introduced.

The top-right and bottom-left groups contain viruses where some of them are half-frozen. In the same rows as the crates, each has a shell and a rotating cannon (one of them half-frozen).

  1. The 8 coins in both row #6 and column #8.
  2. The 5 coins in both row #4 and column #6.

The ninth-revealed row contains dust clouds and a half-frozen virus, where the dust clouds contain viruses, an exploder and a couple of ice fans. The last-revealed row contains half-frozen viruses in between two muddy coins.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 10 lines are introduced.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain the Parabomb assistant upon completing this stage.

The ninth row is the only row where the viruses are not encased in ice. The three middle columns contain the keys and doors, and two dust viruses are below each of the doors.

The middle columns contain viruses surrounded by cages, most of them containing Brick Blocks, and the corresponding lock block is in the middle column below the cages. Between the middle ring of cages, there are columns of fully frozen viruses and pop cannons. Columns of purple coins (caged and uncaged) are next to the columns of fully frozen objects.

Purple coins are unlocked in the following sequence:

  1. The 2 coins in row #6.
  2. The 2 coins in row #5.
  3. The 2 coins in row #4.
  4. The 2 coins in row #3.
  5. The 2 coins in row #2.

Of the Brick Blocks, columns #3, #4, #6 and #7 are Coin Blocks, one of the blocks in each row of two of Brick Blocks contain Capsule Plus, while the rest contain hidden viruses.

The single virus coin is in the middle of the stage, where it is covered by all three colors of cages.

The exploder in the upper-left half is half-frozen. The upper-left half also contains Brick Blocks. The dust clouds all contain rotating cannons.

The top-left and top-right corners contain Brick Blocks surrounded by exploders, where each group of blocks contains a lock block inside and viruses in the rest. Large virus coins are behind each of the group of cages and the Brick Blocks surrounding the exploders, while the small virus coins are behind the caged viruses in L-shapes. A green lock block is in the middle column in between the Empty Blocks.

Muddy coins are in between the groups of blocks in the corners, while exploders are found in the gaps in between the blocks and muddy coins, where the ones near to the corners are half-frozen. Rows or columns of three floatie viruses are found near the groups of blocks near the corners.

In this stage, the barrels are arranged in an eight-pointed star, where each point consists of three barrels. The barrels mostly contain purple coins (caged and uncaged), with the remaining ones containing keys and doors. Rows of keys and doors in cages are found around each of the corners, where half-frozen viruses are also found. The lock blocks are also half-frozen and are near the middle-left and middle-right sides.

Purple coins are unlocked in the following sequence:

  1. The 3 coins in the bottom three barrels in the middle column.
  2. The 3 coins on the leftmost barrels in each of rows #6 to #8.
  3. The 2 coins in the second and third barrels in row #5.

The handy bottles are labeled with different objects, where the top-left one contains skill chargers, the top-right one contains pop cannon, the bottom-left one contains shells and the bottom-right one contains exploders.

Medium-sized muddy coins are found at the beginning of the stage, while muddy coins of all sizes are found starting from the second-revealed row. The keys are in the first-revealed row and the doors are in the last-revealed row.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 9 lines are introduced.

Among the barrels, the ones in the topmost row contain doors, the ones in the ninth row contain keys while the rest are empty.

Within each row of Brick Blocks on both sides, each of them has one purple coin, one barrier, and two viruses. All the barriers start out inactive after they are freed from the blocks. Floatie viruses are found above and below the Brick Blocks in the leftmost and rightmost columns.

Purple coins are unlocked in the following sequence:

  1. The 14 coins inside the Brick Blocks.
  2. The 7 coins in the middle column.

Viruses and half-frozen viruses are found next to the large-sized muddy coins, while viruses and bubbles (containing exploders and viruses) are found near the medium-sized muddy coins.

Those cages have virus coins behind them. The gaps in between the crates, colorful crates, and cages are filled with purple coins and lock blocks.

Purple coins are unlocked in the following sequence:

  1. The 6 coins in rows #1, #4 and #7.
  2. The 9 coins in rows #3, #6 and #9.

In the final stage of this world and starting with 36 capsules, the objective is to eliminate all light-blue viruses (22) and collect all muddy coins (20). The only colors available in this stage are red and light-blue.

At the beginning of this stage and the first-revealed row, in addition to several small muddy coins and three medium-sized muddy coins, there are crates arranged in columns of two or four, ans well as light-blue viruses in rows and columns of two or three. Additionally, the red viruses are all fully frozen. The second-revealed row consists of floatie viruses where the light-blue ones are inside bubbles. The third- and fourth-revealed rows contain three more medium-sized muddy coins, while the fifth- to seventh-revealed rows contain the only large-sized muddy coin. More crates are found in the fourth- to sixth-revealed rows, as well as the last-revealed row. The sixth- and eighth-revealed rows contain light-blue floatie viruses inside bubbles, while the seventh-revealed row contains fully-frozen red viruses. A shell is found in the middle of the last-revealed row.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 8 lines are introduced.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 29. Upon completion, players earn a free Staff Ticket.

Empty Blocks are found in the second to fifth-revealed rows, and a bubble containing a shell is found in the last-revealed row.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced.

Completing this stage awards the player 1000 coins and a Staff Ticket.

Bubbles containing viruses are arranged in two triangular groups at the top-left and the bottom-right, while the rest of those rows are filled with normal viruses. The keys and doors are in all columns except for the middle column. The last-revealed row consists of Empty Blocks.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 4 lines are introduced.

Completing this stage awards the player 1000 coins and Dr. Lemmy.
