World 27 is a cherry blossom world in Dr. Mario World similar to World 14, except it takes place at night, where Peach's Castle appears in the second area. The first area is available as of 11 PM PT on Apr. 7, 2021[1], while the second area, challenge stages and special stage are available as of 11 PM PT on Apr. 21, 2021[2].

When an area is cleared, Toads appear. The viruses featured in the regular stages are red, yellow and light-blue.

Starting with 28 capsules, the objective of this stage is to eliminate all viruses (7), collect all virus coins (10) and collect all keys (3).

In this stage, the exploders encased in one layer of ice and Brick Blocks (all containing floatie viruses) are split into four quadrants where crates are found in the vertical lines and colorful crates are found in the horizontal lines. In the leftmost, middle, and rightmost columns, there are keys, doors and half-frozen floatie viruses.

Starting with 32 capsules, the objective of this stage is to eliminate all viruses (26) and collect all purple coins (36).

In this stage, barrels in groups of four (2 x 2) are arranged in a checkered pattern, in which they contain purple coins (caged and uncaged). The gaps are filled with viruses, shells and pop cannon encased in ice, with the viruses closer to the top-left corner being fully frozen.

Purple coins are unlocked in the following sequence:

  1. The 4 coins in column #9.
  2. The 16 coins in columns #1, #2, #5 and #6.
  3. The 16 coins in columns #3, #4, #7 and #8.

In this stage, viruses are found in the even-numbered rows up to the eighth row, where half of them are inside bubbles. Bubble machines are in rows #1, #3, #7 and #9 where they are either above or below viruses. The rest of the rows are occupied by empty dust clouds.

Starting with 23 capsules, the objective of this stage is to eliminate all viruses (16) and collect all keys (14).

In this stage, the rotating cannons and the middle exploder form a cross pattern, and the rotating cannons start out by aiming at each other in four squares. In the middle column, there is a pop cannon and an exploder. Besides the keys and doors, the stage is also occupied by floatie viruses and colorful crates.

Starting with 33 capsules, the objective of this stage is to eliminate all viruses (24) and collect all coins from Coin Blocks (24) and collect all purple coins (24).

In this stage, the even-numbered rows up the eighth row contain Coin Blocks and purple coins (caged and uncaged) inside colored cages. The middle column and the rows in between the Coin Blocks and caged purple coins contain bubbles, where some of them are empty but the ones that aren't empty contain viruses and floatie viruses, as well as the lock blocks which are only in the middle column. Pop cannons are found in the first and ninth rows, where there are also normal viruses. An extra crate is found in the middle of the topmost row.

Starting with 30 capsules, the objective of this stage is to eliminate all viruses (26) and collect all keys (9). The only colors available in this stage are red and light-blue.

In this stage, the rightmost column contains a pop cannon and dust clouds covering shells. The dust clouds are in the same rows as the barrels, where the ones in the odd-numbered columns have fully frozen keys and doors above and below the group. The barrels contain rotating cannons, floatie viruses, normal viruses, keys, and doors, in addition to several empty ones.

In this stage, the yellow cages containing viruses from the number 99 that is underscored by a row of red cages containing viruses. The middle column contains bubbles containing two rotating cannons, a Brick Block, a yellow lock block, and an exploder. A red lock block is surrounded by the viruses in the left "9" and Brick Blocks. The Brick Blocks in columns #1, #4, #6 and #9 contain hidden viruses.

In this stage, the middle three columns contain rows of purple coins (caged and uncaged) with rows of fully frozen viruses in between. The second and eighth columns contain keys and doors where each pair has a clouded floatie virus in between. The other columns contain barriers and electric exploders.

Purple coins are unlocked in the following sequence:

  1. The 6 coins in rows #1 and #3.
  2. The 6 coins in rows #5 and #7.
  3. The 3 coins in row #9.

In this stage, the normal viruses and bubbled crates are arranged in a cascading pattern alongside the bubble machines and the fully frozen viruses below them. The topmost row contains fully frozen Brick Blocks (all containing coins).

Starting with 30 capsules, the objective of this stage is to eliminate all viruses (40), collect all purple coins (12) and collect all keys (5).

In this stage, the three leftmost and three rightmost columns contain viruses and colorful crates in rows except for the last three revealed rows, which contain keys, purple coins, and doors. The fourth and sixth columns contain viruses and red cages containing viruses, crates, and pop cannons. Directly above the caged pop cannons are Brick Blocks containing hidden exploders, as well as caged coins above those Brick Blocks. The middle column contains a key and bubbles where one of them contains a door and another contains a lock block.

Purple coins are unlocked in the following sequence:

  1. The 8 coins in the three leftmost and three rightmost columns in the last two revealed rows.
  2. The 4 coins in columns #4 and #6 in the last two revealed rows.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 6 lines are introduced.

In this stage, the first nine rows are occupied by Brick Blocks except for a rotating cannon in the bottom-right corner. In addition ot viruses and Coin Blocks, the other Brick Blocks contain exploders, rotating cannons, and electric exploders.

Starting with 25 capsules, the objective of this stage is to eliminate all viruses (34), collect all purple coins (21) and collect all keys (13). The only colors available in this stage are yellow and light-blue.

In this stage, the viruses are arranged in a cross formation, where the gaps at the top and sides contain purple coins (caged and uncaged) and doors in bubbles, while keys are in the gaps at the sides and the bottom. A pop cannon is also found in the bottom gap.

Purple coins are unlocked in the following sequence:

  1. The 9 coins in the first three rows.
  2. The 12 coins in the two leftmost columns and the two rightmost columns.

In this stage, the two leftmost and two rightmost columns contain Brick Blocks checkered with keys and bubbles containing doors and viruses, with the columns next to them closer to the center containing colorful crates. Of the Brick Blocks, the ones in columns #2 and #8 contain hidden viruses. Empty Blocks are located in the topmost row in the three leftmost and three rightmost columns. The middle column contains bubbles in between the key and door, while the columns next to it contain bubble machines with a few viruses in between them.

In this stage, each of the virus coins is covered by caged exploders and their surrounding objects. The viruses and lock blocks are the same color as the exploders adjacent to them, while the colorful crates are differently colored. The group in the top-middle also contains two empty cages.

Starting with 25 capsules, the objective of this stage is to eliminate all viruses (16), collect all virus coins (6) and collect all purple coins (32).

In this stage, the purple coins (caged and uncaged) are arranged in rows next to Empty Blocks, where the ones from row #1 to row #7 are frozen. The barrels in the same columns as the Empty Blocks also contain caged coins, while the rest contains floatie viruses. A pop cannon is located in the leftmost column. Among the dust clouds, the ones in the leftmost column contain shells, the ones in the rightmost column contain Capsule Plus (starting from +1), and the ones in the middle column contain shells in the odd-numbered rows and exploders in the even-numbered rows. The first row contains three handy bottles for exploders, shells, and pop cannons respectively.

Purple coins are unlocked in the following sequence:

  1. The 3 coins in row #9.
  2. The 3 rightmost coins in row #8.
  3. The 3 leftmost coins in row #7.
  4. The 3 rightmost coins in row #6.
  5. The 3 leftmost coins in row #5.

Starting with 15 capsules, the objective of this stage is to eliminate all viruses (56) and collect all coins from Coin Blocks (24).

In this stage, the objects that are not caged form a path starting from a Capsule Plus (starting from +4) that ends with a half-frozen virus, and in the way are viruses and lock blocks, many of them being half-frozen. All the sets of cages each contain one Capsule Plus, which start from +6, +8, and +12 for light-blue, yellow and red cages respectively. Two rows of Brick Blocks (all containing coins) are in the red and yellow cages, while the rest of the cages contain viruses.

Starting with 9 capsules, the objective of this stage is to eliminate all viruses (27) and collect all virus coins (18).

In this stage, half-frozen viruses in groups or two or have a gap in between are found starting from row #4 and below, while barrels and exploders are found starting from row #4 and above. One of the barrels contains an exploder while the rest are empty. Additional viruses are located in the first two rows, with the ones in row #2 being half-frozen.

Starting with 30 capsules, the objective of this stage is to eliminate all viruses (20), break all colorful crates (11) and collect all purple coins (29).

In this stage, bubbles containing viruses and a couple of exploders are surrounded by purple coins (caged and uncaged) in two groups. The top-right contains a group of colorful crates where an exploder is surrounded by it and a pop cannon is in the same column as that exploder.

Purple coins are unlocked in the following sequence:

  1. The first three coins in column #6 and the first coin in column #7, for a total of 4 coins.
  2. The last coin in column #6 and the last three coins in column #7, for a total of 4 coins.
  3. The middle two coins in column #4 and all the coins in column #5, for a total of 7 coins.
  4. The first coin in column #2, all the coins in column #3 and the first and last coin column #4, for a total of 8 coins.
  5. The last coin in column #2 and all the coins in column #1, for a total of 6 coins.

In this stage, the even-numbered rows within rows #2 to #8 contain half-frozen viruses and clouded shells, where each of the rows has a virus that has a key and door in between. The odd-numbered rows ending in row #9 contains crates in columns #2 and #8, ice fans in columns #3, #5, and #7, as well as Brick Blocks (all containing coins) in between the ice fans.

The last stage of the first area and starting with 40 capsules, the objective of this stage is to eliminate all viruses (27) and collect all virus coins (26) and collect all keys (24).

In this stage, there are three sections where each section contains keys and doors among the Brick Blocks. The first section is ranged at the beginning of the stage and the first-revealed row has nine pairs of keys and doors where only one key and seven doors are inside Brick Block. The section also contains exploders where most of them are inside dust clouds and Brick Blocks that contain doors, a red lock block, and viruses. The second section is ranged from the first row to the fifth-revealed row, has seven pairs of keys and doors all hidden inside Brick Blocks. In addition to that, the section also contains electric exploders in red cages and Brick Blocks containing viruses, a light-blue lock block, and caged coins. The third and last section is ranged from the fifth-revealed row to the last-revealed row and contains eight pairs of keys and doors. In addition to that, the section also contains pop cannons in light-blue cages, Empty Blocks, and Brick Blocks containing viruses and caged coins.

Purple coins are unlocked in the following sequence:

  1. The 2 coins in row #1.
  2. The 7 coins in rows #2 to #5, all inside Brick Blocks.
  3. The 2 coins in the fifth-revealed row.
  4. The 6 coins in the second, third, and fourth-revealed rows.
  5. The 9 coins in the sixth to tenth revealed rows.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 12 lines are introduced.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain the Stingby assistant upon completing this stage.

In this stage, each of the virus coins is covered by a door with its surrounding barrels and fully frozen viruses. The middle, leftmost and rightmost columns, as well as the topmost row and the fifth row contain crates. The crates at the top and sides have electric exploders in between while the crates in between the virus coins have keys and doors in between them.

In this stage, the barriers are in two groups where they are in between crates above and below them. Colorful crates are arranged in a checkered pattern in between the topmost row and the topmost bubbles and crates.

In this stage, purple coins are found in all columns except for the fourth and sixth columns, where the ones in the second and eighth columns are caged and inside bubbles, and also located in between the keys and doors alongside the lock blocks. Brick Blocks are in the three middle columns and contain floatie viruses. Additional electric exploders are located in the ninth row.

Purple coins are unlocked in the following sequence:

  1. The 22 coins in the odd-numbered columns.
  2. The 8 coins in columns #2 and #8, all inside bubbles.

In this stage, the colorful crates and viruses are arranged in four "V" patterns, with exploders below the lower viruses in the three leftmost and three rightmost columns that are also directly above Brick Blocks (all containing coins). The color of the objects in each column is like-colored. Below each column of objects, there are crates, with the fourth and sixth columns specifically having two crates.

Starting with 24 capsules, the objective of this stage is to eliminate all viruses (26) and collect all coins from Coin Blocks (36).

In this stage, there are six groups of ice where each contains viruses arranged in rotated "L" patterns. In between the groups of ice are Brick Blocks (all containing coins). Directly below the groups of Coin Blocks and ice are colorful crates, and pop cannons are directly below the lower colorful crates in the third, sixth, and ninth columns.

In this stage, the leftmost, middle, and rightmost columns contain fully frozen keys and doors, with the leftmost and rightmost columns containing exploders and shells in between. Those shells are in the same row as fully frozen viruses that are in between rows of crates. Beside the leftmost and rightmost keys, there are exploders, crates, and Brick Blocks (the middle ones containing hidden exploders). In between the fully frozen doors, there are Brick Blocks that contain hidden viruses.

In this stage, the three leftmost and three rightmost columns contain rows of purple coins (caged and uncaged), pop cannons, and barrels containing viruses and lock blocks. The middle three columns contain caged floatie viruses, a caged exploder, and Brick Blocks containing hidden caged coins.

Purple coins are unlocked in the following sequence:

  1. The 12 coins in rows #4 and #6 that are not inside Brick Blocks.
  2. The 12 coins in rows #1, #3, #7, and #9 that are not inside Brick Blocks.
  3. The 20 coins in the middle three columns that are inside Brick Blocks.

Starting with 22 capsules, the objective of this stage is to eliminate all viruses (24) and collect all virus coins (4).

In this stage, there are four large virus coins each covered by two exploders surrounded by Brick Blocks and barriers, where all the Brick Blocks contain hidden viruses. In between the groups are ice fans, and an exploder is in the middle that is adjacent to each group's corner.

Starting with 35 capsules, the objective of this stage is to eliminate all viruses (29) and collect all coins from Coin Blocks (30). The only colors available in this stage are red and light-blue.

In this stage, the bottom-most row of objects contains a clouded shell, a row of ice cubes, and a clouded floatie virus. Every even-numbered row and odd-numbered revealed row contains crates and dust viruses, and except for the fourth row, Coin Blocks. Every odd-numbered row and even-numbered revealed row contains ice cubes in between floatie viruses.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced.

The capsule amount is increased from 30 to 35 since June 3, 2021. Besides that, this stage is otherwise the same.

Starting with 31 capsules, the objective of this stage is to eliminate all viruses (40) and collect all keys (8).

In this stage, there are four groups that each consists of four quarters: one quarter consists of barriers, two quarters each have a half-frozen exploder and three half-frozen viruses (all like-colored) and one quarter has two rows of two viruses (both rows with different colors). Two of those groups are in between the keys and doors.

In this stage, the barrels and Brick Blocks are arranged in a winding path. All the barrels contain purple coins (caged and uncaged) while the Brick Blocks contain hidden exploders. Within each of the bends of the path, there is a column of crates in between two columns of floatie viruses. The keys and doors are in the leftmost, middle, and rightmost columns, and in between the first two doors and the last two keys are rows of crates.

Purple coins are unlocked in the following sequence:

  1. The coins (all inside barrels) in the leftmost and middle columns, as well as the first two coins in the rightmost column, for a total of 8 coins.
  2. The coins (all inside barrels) in row #3 and the last two in row #8, for a total of 4 coins.

In this stage, the corners consist of half-frozen exploders at the corners of the groups of viruses in cages. The middle consists of a lock block surrounded by partially damaged barrels. The Brick Blocks that contain coins are in the middle column, row #4 and row #6. The other Brick Blocks contain hidden viruses.

  1. The first two coins in both rows #7 and #8, for a total of 4 coins.
  2. The first two coins in both rows #5 and #6, for a total of 4 coins.
  3. The last two coins in both rows #7 and #8, for a total of 4 coins.
  4. The last two coins in both rows #5 and #6, for a total of 4 coins.
  5. The 3 coins in row #3.

Starting with 25 capsules, the objective of this stage is to break all crates (17) and collect all purple coins (13).

In this stage, the stage is scattered with ice fans, crates, purple coins, exploders, electric exploders, and a shell. Many of the objects are frozen, but none of the shells and crates are frozen.

Starting with 33 capsules, the objective of this stage is to eliminate all viruses (35).

In this stage, the Brick Blocks are placed in a checkered pattern alongside the colorful crates and empty cages. All but one of the Brick Blocks contain hidden viruses, with the lock block hidden inside the Brick Block in the middle of the fifth row. The ninth row contains electric exploders.

In the final stage of this world and starting with 42 capsules, the objective is to eliminate all viruses (36), collect all coins from Coin Blocks (30) and collect all keys (9).

At the beginning of this stage, there are columns of half-frozen viruses surrounded by Brick Blocks, colorful crates, and keys, where the columns of Brick Blocks are in between keys and doors, and electric exploders are found below the other Brick Blocks. In the next three revealed rows, there are fully frozen doors and a few fully frozen viruses alongside normal viruses, keys, empty barrels, and an exploder. The three following revealed rows contain colorful crates in the middle three columns, and of those rows, the first and third of them have Coin Blocks while the second of them contain empty barrels. The last-revealed row contains a crate in the middle in between two rotating cannons, while the rest of the row contains barrels containing viruses and exploders.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 28. Upon completion, players earn a free Staff Ticket.

The capsule amount is increased from 30 to 42 since June 3, 2021. Besides that, this stage is otherwise the same.

Within a 43 second time limit, the objective of this stage is to eliminate all blue viruses (13). The only colors available in this stage are blue and green.

In this stage, five of the blue viruses are surrounded by colorful crates where the middle one is inside a barrel while the rest of those are inside bubbles. Directly below each of those groups are additional viruses in which only the middle one does not have an exploder below it. Additional viruses are found in between Empty Blocks at the top.

Completing this stage awards the player 500 coins.

Within a 38 second time limit, the objective of this stage is to eliminate all pink viruses (24). The only colors available in this stage are blue and pink.

In this stage, there are pink cages within rows #4 to #9 where some of them contain pink viruses. Blue viruses inside bubbles are found among the cages. In the middle column, there are normal viruses. The first three rows contain exploders surrounded by barrels, where they contain viruses except for the one in the middle of the first row, which instead contains a lock block.

Completing this stage awards the player 500 coins.

Within a 43 second time limit, the objective of this stage is to eliminate all green viruses (9). The only colors available in this stage are green and pink.

In this stage, the fully-frozen viruses form a maze in which the walls are formed by crates below the third row. In the middle of the topmost row, there is a green floatie virus in between Empty Crates.

Completing this stage awards the player 1000 coins and a Staff Ticket.

Starting with 25 capsules, the objective of this stage is to eliminate all viruses (48) and collect all keys (7).

In this stage, the middle three columns contain keys, where the left and the right of these have Brick Blocks containing viruses in between and empty dust clouds in the middle. Their corresponding doors are found in the third-revealed row. To the left and right of those columns are viruses and colorful crates in a partial checkered pattern. Additional keys are found in the second and third-revealed rows where their corresponding doors are in the last-revealed rows. More empty dust clouds are in the fourth-revealed row.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 5 lines are introduced.

Completing this stage awards the player 1000 coins and Dr. Fire Mario.
