Nurse Peach is Dr. Mario Nakamatsu's assistant and, just like the doctor, she is a different version of Princess Peach that exists alongside the actual character, who appears in some stories of the Super Mario Kodansha manga. Compared to the Princess Peach seen in the manga, she is easily distinguished not just by the nurse outfit, but also by her height and hairstyle, identical to Peach's appearance in the games. She was later replaced by Kinoppe, the doctor's granddaughter, as his assistant. Princess Peach did don the nurse outfit and role during Dr. Mario's events and in one brief instance in Super Mario Land 2 - 6-tsu no Kinka 2.

Nurse Peach appears to be, like the Doctor, another counterpart of Princess Peach that comes from the future and moved into the present time, working as a receptionist in the Mushroom Kingdom Hospital. She is briefly seen in Super Mario Kart 1 as the hospital receptionist, but has a bigger role in Super Mario Kart 3. In chapter 1, she still works as the receptionist, but after Mario and the other pilots of the Super Mario Cart Rally passed through the hospital, destroying it, Dr. Mario and Nurse Peach are held accountable for the damages, and the medical board transfers them to the medical clinic outside Safari Course. Mario pays them a visit in chapter 4, and once again, followed by the other pilots, the clinic is destroyed. Furious, Dr. Mario and Nurse Peach decide to follow them and get the money prize. They take their kart, but then they crash into Wario and Bowser. While Nurse Peach regains consciousness, Wario captures the doctor and replaces him to follow Mario and hinder him from saving Luigi from poisoning. Nurse Peach follows the "doctor's" instructions to stop the heroes, until she discovers the truth, following the heroes for the rest of the race.

Judging by how Peach talks and refers to the doctor, their relationship seems to be strictly professional.
